$50 initiation fee (1-time only; waived if the organization has a current WisHHRA member)
$50 annual membership dues for 1 member
$80 annual membership dues for 2-3 members
$150 annual membership dues for 4 or more members
New members must complete an application for membership. Interested members can either download the form below, complete and send to the office with proper payment; or complete the online application below. Forms must be completed in their entirety; incomplete forms may be held until missing items are completed.

Current and former members are invited to renew their membership online, by calling the office or by requesting an invoice via email.
Members may also join as a group. Group members must be physically located on the same campus (i.e., hospital/clinic campus) to qualify for group rates and be submitted on the same application or renewal. Contact the WisHHRA office if you have any questions or would like more information.
